Why most test cases are useless
You've seen this test case before. Maybe you wrote it. I know I have.
TC-042: Verify login works.
Steps: Enter username. Enter password. Click login.
Expected result: User is logged in.
That's not a test case. That's a wish. It tells the tester nothing about what username to use, what password to enter, what "logged in" actually looks like on screen, or what should happen if the credentials are wrong. It's a reminder that login exists, dressed up as quality assurance.
And yet, this is what most test suites look like. Hundreds of lines in a spreadsheet, each one a vague gesture toward some feature, each one giving the team false confidence that things have been verified.
The false confidence problem
When you have 300 test cases and they all "pass," it feels like your release is solid. But if those test cases are vague, all you've really confirmed is that a tester clicked around for a while and nothing caught fire. You haven't confirmed specific behavior. You haven't confirmed edge cases. You haven't confirmed that the system handles bad input gracefully.
I've watched teams ship bugs that were "covered" by test cases. The test case said "Verify payment processing works." The tester processed one payment with a Visa card for $10. The bug was that refunds over $500 on Amex cards threw a server error. The test case technically passed. The customer was not impressed.
Why this keeps happening
The root cause is almost always the same: test cases are written from memory, not from specifications.
A product manager writes a requirements document. Maybe it's a BRD, maybe it's a user story, maybe it's a Slack message that says "we need login." That document gets reviewed, approved, and filed somewhere. Then, weeks later, a tester sits down to write test cases. They don't go back to the requirements. They open the app, look at the login screen, and write test cases based on what they see.
This means the test cases only cover what's already been built, not what was supposed to be built. You're testing the implementation, not the requirement. If a developer forgot to implement a requirement, your test case won't catch it — because you forgot it too.
What good test cases actually look like
A useful test case is specific enough that two different testers would execute it the same way and get the same result.
Instead of "Verify login works," try:
TC-042: Login with valid credentials - Precondition: User account "testuser@example.com" exists with password "Test1234!" and is in active status - Step 1: Navigate to /login - Step 2: Enter "testuser@example.com" in the email field - Step 3: Enter "Test1234!" in the password field - Step 4: Click the "Sign in" button - Expected: User is redirected to /dashboard. The top nav displays "testuser@example.com". The session cookie is set with a 24-hour expiry.
Now you know exactly what to do, exactly what to check, and exactly what "working" means. A junior tester can run this on their first day. An automated test can be written from it directly.
Requirements-driven testing changes the equation
The fix isn't more discipline. Testers are already doing their best. The fix is connecting test cases back to requirements so that every requirement has coverage and every test case traces back to something the business asked for.
When you start with the requirement — "Users must be able to log in with email and password; sessions expire after 24 hours of inactivity; failed logins are locked after 5 attempts" — the test cases write themselves. You need a test for successful login, a test for wrong password, a test for session expiry, a test for account lockout. The requirement tells you what to cover. You're no longer guessing.
This is what traceability gives you. Not a checkbox for auditors, but actual confidence that what was specified is what was tested.
